abstract

background & importance: the aim of this study was to demonstrate the result of transsphenoid microsurgery for cushing disease in our center and comparing the results in details with other articles, to achieve the best decision for patients management in future. case presentation: between 1991 to 2015, 50 patients suffering from cushing disease were operated. before operation, endocrine tests and neuroimaging examinations were done for all patients and all of them were operated via transsphenoid approach. they were followed up in clinic with endocrine tests and imaging in regular intervals. during this study, 50 cushing cases including 47 primary and three secondary tumors were operated. thirty four (68%) tumors were microadenoma and 16 (32%) macroadenoma. a patient died due to postoperative fulminant meningitis (mortality=2%). mean of follow-up was five years. overall, 40 (80%) patients had remission after operation and 9 (18%) patients achieved no remission, three of them had macroadenoma and six microadenoma. during follow-up period, three (6%) patients had recurrence which needed second operation. in secondary tumor and in macroadenoma with cavernous sinus involvement, the result was not good (33% and 0% remission, respectively). conclusion: transsphenoid surgery is a safe and effective first choice method to manage cushing disease compared to other treatment modalities. in recurrent or resistant cases, other modalities could help to manage patients.

Dopamine receptor expression and function in corticotroph pituitary tumors.

The role of dopamine agonist treatment in corticotroph pituitary tumors is controversial. The aim of this study was to evaluate D(2) receptor expression in 20 corticotroph pituitary tumors and to correlate it to the in vitro effect of dopamine agonists on ACTH secretion and the in vivo effect of short-term cabergoline treatment on cortisol secretion. D(2) expression was evaluated by receptor-li...

Low expression of the cell cycle inhibitor p27Kip1 in normal corticotroph cells, corticotroph tumors, and malignant pituitary tumors.

The cell cycle is regulated by a number of inhibitors, including p27Kip1 (p27), which belongs to the kip1 family. By binding to the cyclin/cyclin-dependent kinase complexes, it regulates progression of G1 to S phase in the cell cycle. It has been reported that p27 knockout mice develop multiorgan hyperplasia and intermediate lobe pituitary tumors secreting ACTH. Previously, we and others have b...

Advances in Surgery for Pituitary Tumors

Although benign in nature, pituitary tumors continue to offer many opportunities for improvement in therapy. Goals of treatment include complete removal or ablation of tumor cells, maintenance of normal pituitary function, normalization of elevated hormone levels in endocrine-active tumors, and minimizing adverse effects from therapy. There have been many recent advances related to the surgical...

Biochemical predictors of outcome of pituitary surgery for Cushing's disease.

OBJECTIVE Transsphenoidal surgery (TS) is the primary therapy for Cushing's disease (CD). The aims of this retrospective study were twofold: (i) investigate early and late results of TS for CD, and (ii) evaluate various postoperative tests in order to predict the outcome of TS. METHODS We reviewed the long-term outcome in 79 patients with CD who underwent TS (median follow-up 84 months, range...

Early assessment of outcome of pituitary surgery for Cushing's disease.

OBJECTIVE The prediction of relapse during the early months after transsphenoidal surgery for Cushing's disease remains difficult. We have evaluated the usefulness of the postoperative metyrapone test in this situation. PATIENTS From a retrospective series of 77 consecutive primary pituitary operations for Cushing's disease 29 patients, who also had a metyrapone test at 14 days postoperativel...
